Glaukom, kao hronično oboljenje oka, i dalje predstavlja enigmu i nerješiv problem u većini slučajeva. Međutim, mnogo je istraživanja koja se upravo bave jednim ciljem, a to je pokušaj trajnog snižavanja intraokularnog pritiska i rješavanja glaukoma.

U posljednjim studijama pokazano je da tzv. trabekularni mikro-bajpas stentovi dovode do značajnog snižavanja intraokularnog pritiska kod većine ispitanika, podjednako dobro kao i prostaglandinska topikalna terapija.

Iako antihipertenzivni, odnosno antiglaukomski lijekovi, pomažu većini pacijenata sa glaukomom otvorenog ugla, neki od njih s vremenom postaju intolerantni na kapi, te se javlja ozbiljan problem kontrole pritiska u oku. Za ove pacijente traže se alternativna rješenja, a jedno i ujedno najčešće je hirurgija glaukoma, odnosno implantacija intraokularnih trabekularnih stentova.

U Sjedinjenim Američkim Državama dozvoljena je implantacija trabekularnih mikro-bajpas stentova, ali samo u kombinaciji sa operacijom katarakte. Da bi se ocijenio efekat ovih implantata, urađena je studija na 50 očiju pacijenata kojima je implantiran stent i 48 očiju onih kojima je glaukom liječen prostaglandinskim kapima. U obe grupe pacijenata dijagnostikovan je dekompenzovani glaukom otvorenog ugla, pseudoeksfolijativni glaukom, ili mogućnost za nastanak okularne hipertenzije, a vrijednosti intraokularnog pritiska su se kretale od 21 do 40 mmHg. Prilikom implantacije titanijumskih stentova korištena je klasična hirurška tehnika, gdje su u svako oko stentovi implantirani kroz trabekularnu mrežu u Schlemm-ov kanal radi poboljšanja oticanja očne vodice iz oka.
Tokom perioda praćenja od 2 godine primjećeno je da je smanjenje intraokularnog pritiska bilo veće u grupi pacijenata sa stentom nego kod pacijenata koji su i dalje koristili prostaglandinske kapi; 4 oka u stent-grupi i 2 oka i prostaglandinskoj grupi izgubila su neznatne vrijednosti od svoje najbolje korigovane vidne oštrine na daljinu. Takođe, samo 5 pacijenata iz stent-grupe i 7 iz prostaglandinske grupe bilo je prinuđeno na dodatnu antiglaukomsku terapiju. Nisu zabilježene intraoperativne ili postoperativne komplikacije kod pacijenata u stent-grupi.
Podaci dobijeni najnovijim studijama upravo ukazuju na činjenicu da podjednaka efikasnost hirurgije i medikamentoznog pristupa liječenju glaukoma otvara vrata mogućnosti da hirurgija u skorijoj budućnosti postane metoda izbora za glaukomske pacijenate.
